In the preceding twelve (12) months, Maximum Solutions Physical Therapy and PT Solutions Holdings, LLC has not “sold” Personal Information (as defined by the California Consumer Privacy Act, or the “CCPA”) for any business or commercial purpose. However, the CCPA broadly defines “sale” in a way that may include using targeted advertising services to deliver targeted advertising about our services and programs. We use these advertising services which enable us to provide relevant content to you. Our use of these services may constitute a “sale” as defined by the CCPA of the following categories of information to advertising partners for such advertising purposes: Category A. Identifiers (i.e., Internet Protocol address, device IDs, cookie IDs, and advertising IDs.)
If you are a California resident 16 years of age or older, you have the right to direct us not to sell your Personal Information at any time (the “Right to Opt-Out”). We do not sell the Personal Information of consumers we actually know are less than 16 years of age, unless we receive affirmative authorization from either the consumer who is between 13 and 16 years of age, or the parent or guardian of a consumer less than 13 years of age.
To exercise the Right to Opt-Out, you (or your authorized representative) may submit a “Do Not Sell My Personal Information” request to us by emailing compliance@ptsolutions.com and including your first and last name in the message.
Once you exercise your Right to Opt-Out, Maximum Solutions Physical Therapy and PT Solutions Holdings, LLC will wait at least twelve (12) months before asking you to reauthorize the sale of your Personal Information. However, you may change your mind and opt back in to personal information sales at any time by e-mailing compliance@ptsolutions.com. Consumers who opt-in to Personal Information sales may opt-out of future sales at any time.
You do not need to create an account with us before making a “Do Not Sell My Personal Information” request. We will only use Personal Information provided in an opt-out request to review and comply with the request.
